Requirements:

Before the preliminary screening, applicants are required to prepare and submit documents to the DILG-CLGSC Chairperson thru its National Secretariat:

1. Resume/CV with picture (2x2)
2. Transcript of Records (Certified True Copy of BS Degree and MS, if any)
3. Diploma (Certified True Copy)
4. List of Trainings/Seminars Attended
5. Updated Service Record
6. Certified Actual Duties and Responsibilities
7. Certificate of No Pending Administrative and Criminal Case
8. Certificate of No Pending Nomination in Local and/or Foreign Scholarship Programs
9. Certificate of No Service Obligation from Local and/or Foreign Scholarship Programs
10. Written Consent of Spouse (if married)
11. Endorsement from DILG Regional Director
12. Endorsement from Local Chief Executive (LCE)
13. Performance Rating for the Last Two (2) Rating Periods - Jan. to June and July to December (Certified copy only)

For Accepted Nominees

Additional Documentary Requirements:

1. Clearance from Money & Property Accountability
2. Clearance from Regional Trial Court
3. Sworn Statement from the Nominee that he/she does not have Administrative or Criminal Case (notarized)
4. Copy of Order of Designated OIC while on official travel abroad (for Department Head level only)
5. Request Letter addressed to the DILG Secretary for the Issuance of the nominee's Travel Authority

NOTE:

• Applicants must submit original and duplicate copies of the said documents to DILG-CLGSC Chairperson thru its Secretariat c/o LOCAL GOVERNMENT ACADEMY (LGA), 8F Agustin I Building, F. Ortigas Jr. Road (formerly Emerald Avenue), Ortigas Center, Pasig City
• All nominees will be officially notified of his/her NOMINATION thru respective DILG Regional Offices c/o LGA Regional Coordinators. Upon receipt of NOTICE, nominees shall coordinate and confirm date of interview to the TESDA-NSC, Taguig City
• Documents #12 and 13 must be submitted only when accepted to attended the scholarship program.
• Applicants from Luzon, Visayas and Mindanao must submit two sets of above documents to the DILG-CLGSC Chairperson thru its Secretariat. Duplicate copies of documents must be submitted to the TESDA-NSC, Taguig City
• All nominees will be officially informed of the date of interview thru their respective DILG Regional offices.
